icon Risks of Telehealth Services

The purpose of the Services provided through the Website is to give health care providers and their patients access to additional information which they may or may not choose to utilize in planning medical care. It is not a substitute for primary care and no health care provider-patient relationship is created by use of this Website.

The Services are limited. In some cases, the online nature of the Service will make it difficult to verify a diagnosis (particularly with conditions that rely heavily on direct health care provider-patient interaction for accurate diagnosis). In those cases, the Service provided through the Website will focus on the assessment or recommendation of treatment options. The decision to focus on diagnosis, treatment recommendation(s), or both rests solely with the local treating health care provider.

The Service provided through this Website differs from the diagnostic services typically provided by a health care provider. The medical professionals providing Services through this Website will not have the benefit of information that would be obtained by examining you in person and observing your physical condition. Therefore, the medical professional providing services through this Website may not be aware of facts or information that would affect his or her opinion of your diagnosis. To reduce the risk to you of this limitation, the Company strongly encourages you to discuss the information gained from the Services with your treating health care provider. By deciding to engage the Services of this Website, you acknowledge and agree that you are aware of this limitation and agree to assume the risk of this limitation. Furthermore, you agree and accept that: (i) the information you will receive is limited and provisional; (ii) the Services are not intended to replace a full medical evaluation or an in-person visit with a health care provider; (iii) the medical professional(s) providing Services through this Website do not have important information that is usually obtained through a physical examination; and (iv) the absence of a physical examination may affect the medical professional’s ability to fully understand your condition, disease or injury.

You also agree that the Services will not be used in any legal dispute including but not limited to litigation, arbitration, claim for disability benefits, claim for worker’s compensation and/or malpractice claims without the prior written consent of the Company.

By requesting any service through the website, you authorize us to collect your medical records on your behalf from all facilities that may have such records, store those records, and transmit them to the health care provider or health care providers appropriate for your case.

The Company takes seriously the privacy of your medical records and personal information. Accordingly, we have taken all reasonable restrictions necessary to protect your confidentiality in accordance with all applicable laws.

In addition, you authorize the Company and their respective health care providers, employees, contractors, business associates and/or agents to access and collect your medical records on your behalf from any and all facilities that may have such records, store those records, transmit them to the health care provider(s) or relevant person(s) appropriate for your case, review, research, analyse, discuss and copy your medical and personal information as may be necessary to provide the opinion you request. The use of your medical information will not exceed this stated purpose, unless and until you authorize and direct such additional disclosure(s).

icon Informed Consent of Services

Telemedicine involves the use of electronic communications to enable healthcare providers at different locations to share individual patient medical information for the purpose of improving patient care. Providers may include primary care practitioners, specialists, and/or subspecialists. The information may be used for diagnosis, therapy, follow-up and/or education, and may include, but not limited to, any of the following (i) Patient medical records; (ii) Medical images; (iii) Live two-way audio and video; (iv) Output data from medical devices and sound and video files.

Electronic systems used will incorporate network and software security protocols to protect the confidentiality of patient identification and imaging data and will include measures to safeguard the data and to ensure its integrity against intentional or unintentional corruption. Responsibility for the patient care should remain with the patient's local clinician, if you have one, as does the patient's medical record.

Expected Benefits:

  • Improved access to medical care by enabling a patient to remain in his/her local healthcare site (i.e. home) while the health care provider consults and obtains test results at distant/other sites.
  • More efficient medical evaluation and management.
  • Obtaining expertise of a specialist.

Possible Risks:

As with any medical procedure, there are potential risks associated with the use of telemedicine. These risks include, but may not be limited to:

  • In rare cases, the consultant may determine that the transmitted information is of inadequate quality, thus necessitating a face-to-face meeting with the patient, or at least a rescheduled video consult.
  • Delays in medical evaluation and treatment could occur due to deficiencies or failures of the equipment.
  • In very rare instances, security protocols could fail, causing a breach of privacy of personal medical information.
  • In rare cases, a lack of access to complete medical records may result in adverse drug interactions or allergic reactions or other judgment errors.

By checking the box associated with "Informed Consent", You acknowledge that you understand and agree with the following:

  • I understand that the laws that protect privacy and the confidentiality of medical information also apply to telemedicine, and that no information obtained in the use of telemedicine, which identifies me, will be disclosed to researchers or other entities without my consent.
  • I understand that I have the right to withhold or withdraw my consent to the use of telemedicine during my care at any time, without affecting my right to future care or treatment.
  • I understand the alternatives to telemedicine consultation as they have been explained to me, and in choosing to participate in a telemedicine consultation, I understand that some parts of the exam involving physical tests may be conducted by individuals at my location, or at a testing facility, at the direction of the consulting healthcare provider.
  • I understand that telemedicine may involve electronic communication of my personal medical information to other medical practitioners who may be in other areas, including out of state.
  • I understand that I may expect the anticipated benefits from the use of telemedicine in my care, but that no results can be guaranteed or assured.
  • I understand that my healthcare information may be shared with other individuals for scheduling and billing purposes. Others may also be present during the consultation other than my healthcare provider and consulting healthcare provider in order to operate the video equipment. The above-mentioned people will all maintain confidentiality of the information obtained.

Patient Consent to The Use of Telemedicine

I have read and understand the information provided above regarding telemedicine, have discussed it with my health care provider or such assistants as may be designated, and all my questions have been answered to my satisfaction.

I have read this document carefully and understand the risks and benefits of the teleconferencing consultation and have had my questions regarding the procedure explained and I hereby give my informed consent to participate in a telemedicine visit under the terms described herein.
