Continuity of Care (COC)

Continuity of Care (COC)

We all know that Silos in corporate world is bad; it is counterproductive, slows down innovation and a hindrance to quality results; Yet we are OK with accepting the same silo in medical treatment where either single or a small group of physicians not working together are making life changing decisions on our treatment plan. Why?

KareOptions Continuity of care service make sure that our Doctors are collaborating with patient, patient's family and/or the treating physicians as needed. Continuity of care is a patient-centric framework that addresses gaps in the diagnosis and treatment by providing additional level of expertise, guidance, and monitoring. As part of the service our doctors will be happy to join virtual meeting or attend phone calls on short notice to discuss new findings or help you guide through the critical decision on treatment options. Our service can bring together a dispersed healthcare team and their patients in the following ways:

Consistency and Quality of Care

Same specialist will be engaged throughout the care to ensure that any new information or discovery can be accounted to make sure that patient is still getting the best possible treatment.

Strengthens the doctor/patient relationship

Video or voice-enabled treatment can follow a patient after the initial Second Opinion Consultation allowing clinicians to provide additional clinical counsel to patients and their caregivers. This can strengthen the doctor/patient relationship; this can engage patients in their own treatment and increase patient satisfaction and confidence about the treatment plan.
